Service Agreement & Informed Consent — Laser Hair Removal & Skin Treatments

1. Scope & Who We Are

This Policy applies to personal information we handle about clients, prospective clients, website visitors, and other individuals who interact with us online or in person.
Legal entity: Sense Beauty Studio and Laser, 2751 182 St SW, Edmonton, AB, T6W4V2.
Privacy Officer: Anna Kozachenko (owner, technician).


2. Key Terms

Personal information: information about an identifiable individual (e.g., name, contact, health disclosures you provide, service history, images).

Processing: collecting, using, storing, disclosing, or otherwise handling personal information.

Service providers: third parties we contract for booking/CRM, payment processing, communications, cloud/IT, analytics, etc.

Consent: your permission—express (opt-in) or implied (e.g., when you book and provide info for that service).


3. What We Collect

We collect the minimum information reasonable for the purposes below:

A. Identity & Contact - Name, preferred name, date of birth (when needed), email, phone, address.

B. Booking & Service Records - Appointments, services selected, treatment notes/settings (where applicable), pre/post-care notes, communications.

C. Health Disclosures (You Choose to Share) - Relevant conditions, medications, allergies, skin history—only to assess suitability/safety of cosmetic services.

D. Payments & Transactions - Payment method tokens, invoices, refunds, and receipts (card details handled by our payment processor; we do not store full card numbers).

E. Photos/Video - Before/after images for your private record; any marketing use is opt-in and optional.

F. Technical & Usage Data (Website/App) - Device, browser, IP (general region), pages viewed, time on site, cookies/analytics (see §8).

We may collect information directly from you (forms, email/SMS, in person), automatically (cookies/analytics), or from service providers (e.g., payment confirmations).


4. Why We Use Your Information (Identifying Purposes)

Provide services: bookings, reminders, treatment delivery, pre/post-care, client support.

Safety & quality: screening for contraindications; maintaining accurate treatment records; staff training.

Payments & administration: invoices, receipts, accounting, audits.

Communications: confirmations, reminders, follow-ups, important updates.

Marketing (optional): promos, newsletters—only with your consent (unsubscribe anytime).

Legal/compliance: record-keeping, responding to lawful requests, enforcing policies.

We do not use personal information for purposes beyond those above without your consent (unless permitted/required by law).


5. Our Privacy Principles (PIPEDA & PIPA)

We apply the recognized principles of accountability, identifying purposes, consent, limiting collection, limiting use/disclosure/retention, accuracy, safeguards, openness, individual access, and challenging compliance. Under PIPA, we collect, use, and disclose personal information only for reasonable purposes.


6. Consent Choices

Service delivery: by providing information to book/receive a service, you give implied consent for collection and use for that service.

Sensitive details & marketing: we seek express consent (opt-in) for health disclosures (where required), marketing messages, and any public use of photos/videos.

Withdraw consent: you may withdraw consent to optional uses at any time (e.g., marketing, public photos). This won’t affect processing already completed or required for service/legal reasons.


7. Children/Minors

If you are under the age of majority in Alberta, a parent/guardian must review this policy and provide any required consents during booking and at the appointment.


8. Cookies, Tracking & Analytics

We may use cookies and similar technologies to:

keep you logged in, remember preferences, and improve performance;

understand site usage (e.g., aggregated analytics such as page views, traffic sources).

Your options: manage cookies in your browser; opt out of marketing emails via the unsubscribe link; reply STOP to SMS where supported. Note: essential cookies may be required for site functionality.


9. Disclosures to Service Providers & Others

We share information with trusted service providers under confidentiality and security obligations, including:

Booking/CRM (appointment scheduling, records)

Payments (card processing, fraud prevention)

Email/SMS (reminders, follow-ups)

Cloud/IT & backups

Analytics (aggregated website metrics)

We may also disclose information:

to comply with lawful requests (court orders, regulators);

to protect people’s safety, rights, or property;

in a business transaction (e.g., reorganization/asset sale) subject to statutory requirements.

We do not sell your personal information.


10. Cross-Border Processing

Some service providers may process data outside Alberta/Canada. Where they do, your information may be subject to that jurisdiction’s laws. We take reasonable contractual and organizational measures to protect it.


11. Retention & Destruction

We retain personal information only as long as reasonably necessary to deliver services, meet legal/accounting requirements, resolve disputes, and enforce agreements. When no longer needed, we securely delete or de-identify it in line with our retention schedule.


12. Accuracy

We rely on you to help keep your information accurate and up to date. Please notify us of changes to contact details, health disclosures relevant to services, or preferences.


13. Safeguards (Security)

We use physical, organizational, and technical safeguards appropriate to the sensitivity of the information, including role-based access, staff training, secure systems, and encrypted transport where applicable. No method is 100% secure; if you suspect an issue, contact our Privacy Officer immediately.


14. Your Rights (Access, Correction, Choices)

Subject to limited exceptions under PIPA/PIPEDA, you can:

Access your personal information and obtain an account of its use and disclosures;

Request corrections to incomplete or inaccurate information;

Withdraw consent to optional uses (e.g., marketing, public photos);

Request explanation of our policies and practices related to service providers and cross-border transfers.

How to make a request: contact our Privacy Officer (see §1). We respond within statutory timelines (generally 30–45 days) and may ask for information to verify your identity.


15. Breach Notification

If we experience a privacy breach that creates a real risk of significant harm, we will:

Notify affected individuals as soon as feasible;

Report to the appropriate regulator (e.g., Alberta’s OIPC; Canada’s OPC) as required;

Maintain records of privacy breaches per legal requirements.


16. Complaints & Escalation

If you have a privacy concern, contact our Privacy Officer first. We will investigate and respond.
If unresolved, you may contact:

Office of the Information and Privacy Commissioner of Alberta (OIPC) — regarding PIPA; and/or

Office of the Privacy Commissioner of Canada (OPC) — regarding PIPEDA.
(They provide instructions on how to file a complaint.)


17. Changes to This Policy

We may update this Policy from time to time. Material changes will be posted on this page with an updated “Last updated” date. We encourage you to review this Policy periodically.
